PARTICIPANT OBLIGATIONS

Participant agrees to participate in induction activities as required by the institution, which may include assigned mentoring sessions, professional development meetings, observations, and submission of program artifacts. Required activities and deliverables are described below or attached as an addendum.

SERVICE OBLIGATION AND REPAYMENT

In consideration of the stipend, Participant agrees to remain employed and in good standing with the Institution for a minimum of academic year(s) following program completion (Service Obligation Period). If Participant resigns, is dismissed for cause, or otherwise separates from employment prior to completion of the Service Obligation Period, Participant shall repay the stipend on a pro rata basis as described below.

TAX WITHHOLDING AND REPORTING

Stipend payments are subject to applicable federal, state, and local tax withholdings and reporting requirements. Participant acknowledges that the Institution will withhold payroll taxes where required by law and will report the stipend on applicable wage and tax forms.

TERMINATION; DEFAULT

Institution may withhold, suspend, or require repayment of stipend funds if Participant materially breaches this Agreement, fails to complete required induction activities, is subject to disciplinary action related to professional conduct, or otherwise fails to satisfy eligibility conditions. Failure to repay amounts when due shall constitute a debt owed to the Institution and may be recovered consistent with applicable policy and law.

CONFIDENTIALITY; RECORDS

Participant consents to the Institution maintaining records of participation and stipend disbursement, and authorizes the Institution to use such records for program evaluation and internal audit. Participant shall not disclose personally identifiable information regarding mentors, evaluators, or students obtained through induction activities except as required by law or institutional policy.

All notices required or permitted under this Agreement shall be in writing and delivered to the addresses set forth below. Notices to Institution:

MISCELLANEOUS PROVISIONS

This Agreement constitutes the entire agreement between the parties with respect to the stipend and supersedes all prior discussions and agreements. Any amendment must be in writing and signed by both parties. If any provision is held unenforceable, the remaining provisions remain in full force. This Agreement is governed by the laws of the jurisdiction where the Institution is located.
