INTERNSHIP ENROLLMENT AGREEMENT

This Internship Enrollment Agreement ("Agreement") is entered into by and between the parties identified below for the purpose of documenting the terms under which the Student/Intern will participate in an educational internship at the Host Organization. The parties acknowledge that this Agreement sets forth the responsibilities, supervision, academic requirements, and legal terms governing the placement.

Supervision, Evaluation and Academic Requirements

Site supervision will be provided by the Site Supervisor identified above. The Host Organization agrees to provide regular feedback and an end-of-term written evaluation suitable for submission to the academic advisor.

Confidentiality, Intellectual Property, and Records

Confidentiality: The Intern shall keep confidential all proprietary, nonpublic, and sensitive information obtained during the internship and shall not disclose such information except as required by law or with prior written authorization of the Host Organization. Upon request, the Intern shall return or certify destruction of confidential materials.

Intellectual Property: All inventions, works of authorship, designs, discoveries, improvements, and materials created by the Intern in the course of the internship that relate directly to the business or research of the Host Organization shall be the property of the Host Organization. The Intern hereby assigns to the Host Organization all right, title, and interest in such intellectual property and agrees to execute documents reasonably necessary to effectuate such assignment.

Safety, Insurance, and Liability

The Intern acknowledges that participation in the internship may involve certain physical risks. The Intern is responsible for maintaining health and accident insurance. The Host Organization will maintain customary employer liability insurance for its operations; however, the Host Organization is not responsible for the Intern's personal property.

Release and Indemnification: To the extent permitted by law, the Intern agrees to release, defend, indemnify, and hold harmless the Host Organization, its officers, employees, and agents from and against any claims, liabilities, damages, or expenses (including reasonable attorneys' fees) arising out of or resulting from the Intern's negligent acts or omissions during the internship, except to the extent such claims result from the Host Organization's gross negligence or willful misconduct.

Compliance, Background Checks and Termination

The Intern agrees to comply with all applicable Host Organization policies, procedures, and workplace rules, including but not limited to workplace safety, confidentiality, and nondiscrimination policies. The Intern consents to any background checks or health screenings reasonably required by the Host Organization as a condition of participation.

Termination: Either party may terminate this Agreement for any reason upon providing reasonable written notice to the other party. The Host Organization may terminate immediately for cause, including but not limited to violation of workplace rules, unsafe behavior, or breach of confidentiality.

Records, Privacy and Data

The Host Organization and School may collect and retain records reasonably necessary to document internship performance and compliance. The parties will handle personally identifiable information in accordance with applicable privacy obligations. The Intern authorizes the Host Organization to share necessary evaluation and attendance information with the academic advisor or school administration.

Acknowledgements and Certifications

By signing below, the Intern certifies that the information provided in this Agreement is accurate and complete, that the Intern has read and understands the terms and obligations set forth herein, and that the Intern consents to the policies and procedures described above.
