Participation Consent, Assumption of Risk & Release

I, the undersigned parent/guardian or participant, certify that the participant is physically fit to engage in lacrosse and related conditioning. I understand that participation in lacrosse is a contact sport that involves inherent risks including but not limited to bodily injury, concussion, paralysis, or death. I voluntarily assume full responsibility for all such risks, whether known or unknown, foreseeable or unforeseeable.

In consideration for the participant being allowed to engage in lacrosse activities, I hereby release, waive, discharge, and covenant not to sue the school district, school, coaches, team officials, league organizers, volunteers, or other representatives (collectively, the Released Parties) from any and all liabilities, claims, demands, actions or causes of action arising out of or related to any loss, damage, or injury, including death, that may be sustained by the participant while participating in lacrosse or traveling to and from lacrosse events, whether caused by the negligence of the Released Parties or otherwise.

I agree to indemnify and hold harmless the Released Parties from any loss, liability, damage, or cost they may incur due to the participant's participation, including attorney fees and costs incurred in defense of any claims.

Medical Treatment Authorization

If the participant requires emergency medical treatment while participating in lacrosse activities and I cannot be reached immediately, I authorize coaches, team staff, or other representatives to secure medical treatment and ambulance transport as deemed necessary. I authorize licensed medical personnel to administer emergency care and to disclose necessary medical information to treating providers. I understand that I am responsible for any costs associated with such treatment.

Transportation & Event Participation

I acknowledge that some practices, games, or events may require off-site travel. Transportation may be provided by school vehicles, contracted buses, or by parents/volunteers. Conduct, Equipment & Insurance

The participant agrees to follow the rules, codes of conduct, and safety instructions provided by coaches and officials. The participant will use required protective equipment, including helmet, mouthguard, shoulder pads, arm pads, gloves, and proper footwear. The school and coaches are not responsible for replacing lost or damaged personal equipment.

I understand that the participant's personal health insurance is primary for any medical expenses arising from participation, and that the school or league does not guarantee coverage.

Media Release
