All fees are due in accordance with the schedule above. Late payments may incur a late fee and/or suspension from Services until accounts are current. The deposit is non-refundable except where Provider cancels the program. Provider will provide a written statement of account and may withhold grades, certificates, or final reports for unpaid balances.

Attendance, Conduct, and Make-Up Sessions

Participant must attend scheduled sessions punctually and comply with Provider policies and instructor directions. Excessive absences may result in removal from the program without refund. Make-up sessions are offered at Provider discretion and subject to scheduling availability. Participant agrees to notify Provider of anticipated absences in writing.

Medical, Emergency, and Accommodations

In the event of an emergency, Provider is authorized to seek medical treatment for Participant if Provider is unable to contact the parent/guardian. Provider will make reasonable efforts to notify the emergency contact as soon as practicable.

Confidentiality, Records, and Progress Reporting

Provider will maintain records of attendance and progress. Progress reports may be shared with Participant and parent/guardian if Participant is a minor. Provider will maintain confidentiality of records consistent with applicable law. Provider may use aggregated, de-identified data for program evaluation and internal quality improvement.

Consent to Share Progress Reports: I consent to Provider sharing progress reports with the individuals named in this Agreement as appropriate.

Photo and Video Release

Provider may take photographs or video recordings during activities for educational, promotional, or documentation purposes. Participant or parent/guardian may decline such use by checking the box below.

Liability, Waiver, and Indemnification

Participant and parent/guardian (if applicable) acknowledge that participation in life skills training involves inherent risks. To the fullest extent permitted by law, Participant releases, waives, and discharges Provider from liability for injury, loss, or damage to person or property arising out of or related to participation, except arising from Provider's gross negligence or willful misconduct. Participant agrees to indemnify and hold Provider harmless from any third-party claims related to Participant's conduct during the program.

Acknowledgement of Waiver and Indemnity: I have read, understand, and accept the waiver and indemnification terms above.

Cancellation, Termination, and Refunds

Provider may cancel a program for insufficient enrollment or other operational reasons; in such case Provider will offer a full refund of fees paid or credit toward a future program. Participant-initiated withdrawal prior to the first scheduled session may be eligible for a partial refund less a non-refundable deposit. No refunds are provided for sessions attended. Specific refund amounts are determined by Provider policy and the date of written notice of withdrawal.

Termination for Cause and Amendments

Provider may terminate this Agreement and remove Participant from the program for failure to meet standards of behavior, nonpayment of fees, or material breach of this Agreement. Provider reserves the right to amend program policies or schedules; material amendments that materially impair Participant's rights will be communicated in writing and permit withdrawal with refund consistent with the refund policy above.

Governing Law and Dispute Resolution

This Agreement is governed by the laws of the state in which Provider maintains its primary place of business. Disputes arising under this Agreement will be resolved by good faith negotiation between the parties; if unresolved, the parties may pursue mediation or other mutually agreeable dispute resolution procedures prior to filing an action in court.
