Payment, Cancellation & Refund Policy

Enrollment is confirmed upon receipt of payment or employer authorization. Cancellations received in writing at least 7 calendar days prior to the scheduled training date are eligible for a full refund less administrative fees. Cancellations received within 7 calendar days may be assessed a cancellation fee up to 50% of the course fee. No-shows or cancellations on the day of training are not refundable. Rescheduling requests are subject to availability and may incur additional fees.

Acknowledgment of Risk, Waiver and Indemnity

The undersigned Participant (or Parent/Guardian if Participant is a minor) acknowledges that operation of lifting equipment involves inherent risks, including property damage, bodily injury, and death. Participant attests that they are physically able to participate, and have disclosed any medical conditions that could affect safe participation. Participant agrees to follow all instructions, safety policies, and applicable standards provided by the Training Provider and to wear required personal protective equipment.

By signing below, Participant releases, waives, discharges and covenants not to sue the Training Provider, its officers, employees, agents and contractors for liability for personal injury, death, or property damage arising out of participation, except to the extent caused by the gross negligence or willful misconduct of the Training Provider. Participant agrees to indemnify and hold harmless the Training Provider from claims, damages, losses and expenses, including reasonable attorney fees, arising from Participant's negligent acts or omissions.

Completion, Testing & Certificate

Successful completion requires attendance in the classroom and practical demonstration of skills to the Trainer's satisfaction. A written and/or practical assessment may be administered. Upon satisfactory completion, the Training Provider will issue a certificate of completion specifying the equipment and training level. Certificates may be withheld until all fees are paid in full.

Privacy and Records

Participant data and training records will be maintained by the Training Provider for training verification and regulatory compliance. Records will be retained for a reasonable period in accordance with internal recordkeeping policies. The Training Provider will not disclose personal information except as required for invoicing, certification, or as required by law.
