Cancellation and Refunds: If the Student or Guardian cancels enrollment more than 14 days before the program start date, a full refund less any non-refundable registration fee may be issued. If cancellation occurs after program commencement, refunds, if any, will be prorated at the Provider's sole discretion and subject to payment of services already rendered. Provider may terminate services for material breach, nonpayment, or conduct violating program policies; termination does not relieve Student/Guardian of outstanding payment obligations.

Confidentiality, Records, and Data

Records: Provider will maintain records of attendance, assessments, and progress reports. Those records are the property of the Provider but shall be made available to the Student/Guardian upon written request, subject to institutional record retention policies and applicable law governing student records and privacy.

Use of Student Information: Provider may use de-identified aggregate data for program evaluation and improvement. Personally identifiable information will not be disclosed without written consent except as required by law or permitted under applicable privacy statutes.

Liability, Indemnification, and Insurance

Liability: To the maximum extent permitted by law, the Provider's liability for any claim arising under this Agreement shall be limited to direct damages up to the total tuition paid for the Program. Provider shall not be liable for indirect, consequential, special, or punitive damages.

Indemnification: Student/Guardian agrees to indemnify and hold harmless the Provider, its officers, employees, and agents from claims, liabilities, losses, expenses, and damages arising from the Student's conduct, willful misconduct, or breach of this Agreement.

Acknowledgments and Consent

By signing this Agreement, the Student/Guardian acknowledges that they have read and understood the program policies, fee obligations, attendance requirements, confidentiality provisions, and cancellation policy set forth above.

Consent for Treatment: In the event of a medical emergency, the undersigned authorizes the Provider to obtain emergency medical treatment for the Student and agrees to be financially responsible for such treatment.
