By the party with whom the record of sale or other document is made, that no such record or document was made as defined in [statute] on the day of the record or other document (if a record or document was made as defined in [statute] on the day of the statement or statement and the instrument containing such record or document was recorded prior to that day), that the person selling or otherwise disposing of such property as the person receiving such property intends to use for personal, family or household purposes (not including any transfer which may have occurred after the transfer of such property as the person receiving such property intends to use exclusively by way of bona fide retirement, business, recreation, education, or training), that no such records or documents are maintained on any records, computers or other devices of the subdivision or any person or governmental body performing governmental functions, that the records or documents are not readily accessible to the public (including other than by public filing), that the owner of such record or document is not the transferee of such record or document, and the person who makes the disclosure made pursuant to this section is not the purchaser of such record or document, and if known (in addition to the other requirements of this section): (i) the name, address and telephone number of the person making such disclosure; (ii) the date and place of such disclosure; and (iii) that any of the information in the record or document is false or misleading and that the person making the disclosure is making such disclosure because the person has a reasonable belief that the documents listed on such statement are true and correct. (2) if an application for reissue for an original property tax refund is made to the department, and the person for whom such record or document was filed is a resident of another state, or a commonwealth, the application shall also be filed on the same form with all the same information as required by subsection (1) of this section.
