MATH TUTORING AGREEMENT

This Math Tutoring Agreement ("Agreement") is made effective as of between Tutor Name: and Student Name: (if student is a minor, Parent/Guardian executes on behalf of Student).

Fees, Payment & Refunds

Refunds for canceled prepaid sessions will be provided according to the Cancellation and Rescheduling clause below. Tutor shall retain fees for sessions completed. Any disputed charges shall be addressed in good faith between parties.

Cancellation and Rescheduling

A minimum of 24 hours' notice is required to cancel or reschedule a session without charge. Missed sessions without at least 24 hours' notice will be billed at the full session rate. Tutor may reschedule due to illness or emergency and will provide notice as soon as practicable.

Parties acknowledge that repeated cancellations may result in modification or termination of this Agreement under the Termination clause.

Materials, Homework & Academic Integrity

The Tutor will provide instruction, practice materials, and feedback. The Student and Parent/Guardian agree that the Student will complete assigned work and will not submit work that is materially fraudulent. The Tutor does not guarantee specific academic results.

Confidentiality and Records

Tutor will maintain confidentiality of Student records and performance information except as required by law or to protect the Student's welfare. The Tutor may share progress summaries with Parent/Guardian as agreed.

Liability, Indemnification & Governing Law

Tutor will exercise reasonable care in providing services. Tutor is not liable for indirect, incidental, or consequential damages. Parent/Guardian agrees to indemnify and hold harmless the Tutor for claims arising from Student conduct except to the extent caused by Tutor's gross negligence or willful misconduct.

Term and Termination

This Agreement continues until terminated by either party with 14 days' written notice. Tutor may terminate immediately for nonpayment or conduct that materially interferes with instruction. Upon termination, any prepaid, unused sessions will be refunded pro rata less any outstanding charges.

Emergency Contact & Medical Information

Parent/Guardian authorizes Tutor to seek emergency medical treatment for the Student if needed and will be notified promptly.
