Duties and Responsibilities

The Mentor agrees to perform the following duties in a professional manner, consistent with the goals of the Program and applicable laws and policies.

Training, Screening, and Compliance

Mentor acknowledges that successful performance is contingent upon completion of required orientation, training, and screening. Mentor hereby certifies the truthfulness of representations made to Program and consents to the following:

Consent to background checks, fingerprinting, or other screening as required by Program policy or applicable law.

Agreement to attend mandatory training, including child protection and safety training.

Confidentiality and Data

Mentor shall maintain the confidentiality of personally identifiable information and educational records in accordance with applicable law and Program policy. Mentor will not disclose sensitive information about the mentee or family except when disclosure is required by law or necessary to protect the health or safety of the mentee.

Mentor acknowledges mandatory reporting obligations and will immediately report any suspicion of abuse, neglect, or other safety concerns to the Program's designated official and appropriate authorities.

Expenses and Reimbursement

Reimbursement for pre-approved expenses is governed by Program policy. Mentor must obtain prior written approval for reimbursable expenses and submit receipts in accordance with Program procedures.

Termination

Either party may terminate this Agreement at any time, with or without cause, upon written notice to the other party. Program reserves the right to immediately suspend or terminate Mentor's participation for violations of policy, failure to complete required screening or training, or conduct that jeopardizes the well-being of participants.

Representations, Indemnity, and Liability

Mentor represents that all information provided to Program is true and complete. Mentor agrees to indemnify and hold harmless the Program and its officers, employees, and agents from and against claims arising from Mentor's willful misconduct or gross negligence. Mentor acknowledges that Program does not provide professional liability coverage for the Mentor unless expressly agreed in writing.

Records, Reporting, and Evaluation

Mentor agrees to maintain appropriate contact logs and to submit periodic reports required by Program. Mentor consents to participation in program evaluations and to the use of non-identifying data for program improvement and reporting.

Acknowledgment

By signing below, Mentor and Program Representative acknowledge that they have read, understand, and agree to be bound by the terms of this Agreement and that this Agreement constitutes the entire understanding between the parties regarding the mentoring relationship.
