RESPONSIBILITIES & EXPECTATIONS

Mentor agrees to: maintain appropriate boundaries with the mentee; prepare for meetings; communicate progress to program staff; comply with required training and background screening; and immediately report any concerns regarding the safety or welfare of a student.

Mentor Certification of Eligibility: I confirm that I am legally eligible to serve as a mentor, have disclosed any prior convictions or pending charges that would affect my suitability, and will notify the Program of any changes.

I consent to a criminal background check and child safety screening as required by the Program.

CONFIDENTIALITY, DATA, AND MEDIA

Confidential Information: Mentor will safeguard confidential student information and will not disclose educational records or protected personal data except as required by law or as necessary to protect a student's health or safety.

Mentor consents to the Program's use of non-sensitive photographs or quotes for Program publicity when written permission has been obtained from the mentee or guardian.

COMPENSATION & EXPENSES

Relationship: Mentor acknowledges that this engagement is volunteer-based unless otherwise specified below. Any stipend, reimbursement, or other compensation must be documented in writing by the Program.

TERMINATION

Either party may terminate this Agreement with written notice. Program may immediately suspend or terminate a Mentor for violation of policy, failure to complete mandatory requirements, or for issues related to the safety of students.

LIABILITY & INDEMNIFICATION

Mentor agrees to hold harmless and indemnify the Program to the extent permitted by law for claims arising from Mentor's intentional misconduct or gross negligence. The Program will maintain appropriate institutional liability coverage for program activities.

ACKNOWLEDGMENTS

By signing below, the Mentor affirms that the information provided in this Agreement is true and complete, that Mentor will comply with Program policies, and that Mentor understands the mandatory reporting obligations that apply to suspected child abuse or neglect.
