Mentor further agrees to notify the Program Contact in writing at least days prior to any extended unavailability.

Mentor represents and warrants that all information supplied in connection with this enrollment is true and complete. Mentor authorizes the Program to obtain criminal background checks, child abuse registry checks, and reference checks as required by program policy.

Confidentiality and Conduct

Mentor will maintain the confidentiality of student records and personal information in accordance with applicable law and Program policy. Mentor shall not disclose, reproduce, or otherwise disseminate confidential information except to authorized Program staff or as required by law.

Mentor acknowledges mandatory reporting obligations and will immediately report any suspicion of child abuse, neglect, or any conduct that places a student at risk to the Program Contact and appropriate authorities.

Training, Meetings and Recordkeeping

Mentor agrees to complete required training modules prior to substantive engagement and to participate in periodic supervision meetings. Mentor will maintain brief logs of mentoring activities and submit such reports as required by the Program on a regular basis.

Termination; Liability; Indemnification

Either party may terminate this Agreement for any reason upon written notice to the other party. The Program reserves the right to suspend or remove Mentor from the program immediately where continued participation poses a risk to students or violates program policy.

Mentor agrees to indemnify and hold harmless the Program, its officers, employees, and agents from any claims arising from Mentor's willful misconduct, negligence, or breach of this Agreement, except to the extent such claims arise from the Program's gross negligence or willful misconduct.

By signing below, Mentor certifies that the information provided is true and complete, that Mentor will comply with Program policies and applicable law, and that Mentor understands that falsification of information may result in removal from the Program.
