By signing below, I acknowledge and agree that I have been informed of the nature of the ride along activity and accept the inherent risks associated with vehicular travel, including but not limited to traffic accidents, weather-related hazards, and acts of third parties. I voluntarily assume all such risks and release the school district, its employees, volunteers, and the mentor from liability for injury, death, or property damage arising out of or in connection with my participation except to the extent caused by gross negligence or willful misconduct.

I further agree to indemnify and hold harmless the school district, its employees and agents, and the mentor for any claims, costs, or damages arising from my negligence or breach of this agreement. This release shall be binding on my heirs, assigns, and personal representatives.

If the participant is a minor, I authorize school personnel or the mentor to secure emergency medical treatment for the minor if necessary. I understand that I am responsible for any medical or hospital charges incurred on behalf of the minor.

Conduct, Confidentiality, and Photography

The student must comply with all instructions from the mentor and school representatives. The student must not use electronic devices in a manner that distracts the driver. The student shall not disclose confidential information observed during the ride along and shall follow applicable privacy policies and professional standards.
