Refunds: Registration and travel fees are subject to the program's published refund and cancellation policy. Participant acknowledges that nonrefundable costs may be incurred and accepts financial responsibility for any nonrefundable fees as determined by the program administration.

Code of Conduct & Disciplinary Authority

Participant agrees to conduct themselves in a manner that reflects positively on the sponsoring school and program. Prohibited conduct includes, but is not limited to, violation of law, possession or use of illegal substances, harassment, theft, vandalism, and repeated disregard for chaperone or staff directions. The school reserves the right to impose disciplinary action, including immediate dismissal from the conference and removal from overnight housing at the participant's expense, for violations of this code.

I consent to the photographing, audio recording, and video recording of the participant during program activities and grant the school and its agents the right to use such media for educational, promotional, or archival purposes without further compensation. Consent is voluntary and may be revoked in writing to the program director, except where media have already been published.

In consideration of being allowed to participate in Model UN activities, the undersigned (participant or parent/guardian) acknowledges and agrees as follows:

1. Emergency Medical Authorization: If medical treatment is required while participating and a parent/guardian cannot be reached, I authorize program staff and their designated agents to seek and approve emergency medical care and treatment for the participant. I agree to pay all costs associated with such care and to indemnify the school against claims arising from the provision of emergency medical treatment.

2. Liability Release: To the fullest extent permitted by law, I release, hold harmless, and covenant not to sue the sponsoring school, its employees, volunteers, chaperones, or agents from any liability for personal injury, illness, property damage, or other loss arising out of participation in the Model UN program, except for willful misconduct or gross negligence.

3. Accuracy of Information: I represent that the information provided on this form is true and complete to the best of my knowledge. I will notify program staff promptly of any changes to medical or contact information prior to departure.

4. Compliance with Laws and Rules: I agree the participant will comply with all applicable laws, conference rules, and instructions provided by program staff, sponsors, and chaperones. Failure to do so may result in disciplinary action, including removal from the program without refund.
