I, the undersigned parent or legal guardian of the student named above (or the student if age eighteen (18) or older), grant permission for the student to participate in the trip described above. I acknowledge that participation in this trip involves inherent risks, including travel-related risks, and I knowingly assume those risks on behalf of the student.

In the event of illness or injury, I authorize representatives of the school or organization to obtain medical treatment for the student and to consent to any x-ray examination, anesthetic, medical, dental or surgical diagnosis or treatment deemed necessary by licensed medical personnel.
