I further agree to assume responsibility for any liability that may arise out of ordinary negligence or otherwise, with the exception of acts of gross negligence, or intentional, willful or wanton misconduct. I fully understand and acknowledge that risks and dangers are inherent in the aforementioned activity and that said child’s participation in such activities and/or use of the accompanying equipment may result in injury or illness or death or damage to personal property. On behalf of myself, my heirs, assigns, child, and next of kin, I waive all claims for damages, injuries and death sustained to my child that I may have against      arising out of ordinary negligence or otherwise with the exception of acts of gross negligence as indicated above. I warrant that the aforementioned child is in good health and possesses the requisite skills and/or abilities to participate in said activity.
