Student acknowledges that withdrawal may result in academic consequences including, but not limited to, assignment of grades for enrolled coursework, notation of withdrawal on the academic record, termination of assistantship or fellowship appointments, and potential obligations to repay funds or awards under the terms of those awards.

Student certifies that prior to signing this Agreement the Student has been offered the opportunity to consult with the Student's supervisor, program director, and/or a representative in the Office of Graduate Studies and has been advised of the academic and financial consequences of withdrawal.
