Assumption of Risk and Release: I acknowledge that participation in pottery classes involves inherent risks, including but not limited to cuts, burns, inhalation exposure to clay dust, and repetitive motion injury. I voluntarily assume all risks associated with participation. I, on behalf of myself, my heirs, assigns and next of kin, hereby release, indemnify, and hold harmless the studio, its owners, instructors, employees and agents from and against any and all claims, liabilities, costs and causes of action arising out of or related to my participation, except for claims based on gross negligence or intentional misconduct by the studio.

Emergency Medical Consent: In the event of injury or medical emergency, I authorize studio personnel to seek emergency medical treatment on my behalf or on behalf of the enrolled minor. I agree to be financially responsible for any treatment authorized and rendered.

Tuition, Cancellation and Refund Policy: Tuition is due as agreed at registration. If a student withdraws prior to the first class session, a refund less administrative fee may be issued per studio policy. After instruction has commenced, refunds are issued only at the studio's sole discretion and may be pro-rated for documented medical reasons. The studio reserves the right to cancel or reschedule classes; if the studio cancels a class session, a refund or credit for the missed session will be provided.

Materials & Work Ownership: Students are responsible for consumable materials as indicated by materials fee. Finished pieces may be subject to studio handling and kiln firing fees. Unclaimed work left for more than 60 days after final class may be considered abandoned and disposed of unless other arrangements are made in writing.

Conduct and Safety: Students must follow instructor directions, workshop safety protocols, and equipment training requirements. The studio may remove any student whose conduct threatens safety or learning; removal does not obligate refund except at studio discretion.

Photo/Video Release: I grant permission for the studio to photograph or record my image or the image of the enrolled minor for promotional or educational purposes without compensation. I do not grant permission.

Acknowledgment of Policies: By signing below I certify that the information provided is true and complete to the best of my knowledge, that I have read and understand the policies above, and that I have authority to enroll the student named on this form. I understand that failure to provide accurate medical information may affect the studio's ability to respond to an emergency.
