Medical Authorization: In the event of a medical emergency, if the Parent/Guardian cannot be reached, I authorize the school to obtain emergency medical treatment for the student and to transport the student to a medical facility. Authorization: By selecting Autopay or signing below for payment authorization, Parent/Guardian authorizes the school to collect tuition payments pursuant to the selected payment schedule and consents to electronic billing and collection. Parent/Guardian agrees to provide updated payment information when requested.

Attendance & Absence: Tuition is charged for reserved enrollment space. Absences for illness or vacation do not reduce monthly tuition unless otherwise provided in a written fee adjustment agreement. Extended absences may require payment or written agreement to hold space.

Illness & Exclusion: The center follows public health guidance regarding illness exclusion. Parent/Guardian will notify the center of communicable illnesses. The center may require a physician's clearance for return following certain illnesses.

Discipline: The center's discipline policy emphasizes positive guidance. Corporal punishment is prohibited. Parent/Guardian acknowledges receipt of the discipline policy and agrees to support the center's behavioral management procedures.

Late Pickup Fee: Parent/Guardian agrees to pay the published late pickup fee when the student is not picked up by the designated time.

Termination & Withdrawal: To withdraw or reduce enrollment, Parent/Guardian must provide written notice of no fewer than days. The school reserves the right to terminate enrollment for failure to comply with policies or nonpayment of tuition.

Field Trips & Transportation: Parent/Guardian authorizes participation in supervised on-site and nearby off-site activities if field trip consent is provided below. Transportation by authorized staff or contracted carrier will only occur with separate written authorization.

Acknowledgments & Agreements

Enrollment Acceptance: Acceptance of this enrollment agreement by the center confirms a reserved space for the student under the terms set forth herein. The non-refundable deposit secures space and will be applied to the final tuition installment if applicable.

Immunizations: Parent/Guardian certifies that immunization records have been provided and are current in accordance with applicable public health requirements, or that a valid exemption has been submitted. Parent/Guardian will update records when required.

Liability & Indemnification: Parent/Guardian releases the center and its employees from liability for ordinary negligence and agrees to indemnify and hold harmless the center from claims arising from Parent/Guardian's acts or omissions, except to the extent caused by gross negligence or willful misconduct of the center.

Records & Confidentiality: The center will maintain records in accordance with applicable law. The center may share information with emergency medical providers as necessary. Non-confidential directory information may be used for internal communication.

Handbook & Policy Acceptance: Parent/Guardian acknowledges receipt of and agrees to comply with the center's parent handbook and posted policies, including health, safety, and payment procedures.

Certification: By signing below, Parent/Guardian certifies that the information supplied in this Enrollment Agreement is true and complete to the best of their knowledge; that they are the legal parent or guardian of the student named herein; and that they accept and agree to the terms, fees, policies, consents, and authorizations set forth in this document.
