By signing below the Enrollment Party authorizes the Preschool Provider to charge tuition and fees according to the billing schedule above, including late fees and collection costs for delinquent accounts as provided in this Agreement. Enrollment Party agrees to provide payment account information and to maintain valid payment authorization on file.

Late Payment, Withdrawal & Refund Policy

Late Payment: A late fee of $ will be charged if payment is not received within days of the due date. Continuous nonpayment may result in termination of enrollment and referral to collections.

Withdrawal Notice: Enrollment Party must provide written notice of intent to withdraw the student at least days prior to withdrawal. Security deposit handling and any final charges will follow the Provider's stated refund policy below.

Health and Exclusion Policy: The Enrollment Party agrees to abide by the Provider's health policies, including exclusion for contagious illness. The Provider will notify the Enrollment Party of any communicable disease exposures in accordance with applicable public health requirements.

Liability and Indemnification: To the fullest extent permitted by law, the Enrollment Party releases the Provider, its employees, and agents from liability for injury or loss arising from ordinary risks of preschool activities and agrees to indemnify and hold harmless the Provider from claims arising from Enrollment Party's negligence or willful misconduct.

Governing Law and Dispute Resolution: This Agreement will be governed by the substantive laws of the state in which the Provider operates. Any dispute arising under this Agreement shall be resolved by negotiation between the parties and, if unresolved, by binding arbitration in accordance with the Provider's arbitration policy.

Records and Confidentiality: The Provider will maintain student records in accordance with applicable law and Provider policy. The Enrollment Party authorizes release of records to authorized agencies when required by law or as necessary for the child's welfare.

Severability: If any provision of this Agreement is held invalid or unenforceable, the remaining provisions shall remain in full force and effect.
