PRIVATE CLASS ENROLLMENT CONTRACT

This Private Class Enrollment Contract (the "Contract") is entered into by and between the Provider named below and the Student named below for the provision of private instruction under the terms and conditions set forth herein.

REFUND, CANCELLATION & MAKE-UP POLICY

Deposit is non-refundable except where the Provider cancels the class. Student or Guardian must provide written notice of cancellation at least days prior to the first scheduled session to receive a refund of tuition less the non-refundable deposit. After commencement of instruction, refunds for unused sessions will be prorated based on sessions remaining and subject to administrative deduction as set by Provider.

Make-up classes: Provider will permit make-up sessions only where scheduling permits and where absences are notified at least days in advance. Missed classes without prior notice are not eligible for make-up or refund.

ATTENDANCE, CONDUCT & INSTRUCTION

Student agrees to attend scheduled sessions punctually, to comply with instruction and safety directives, and to maintain appropriate conduct. Provider reserves the right to suspend or terminate enrollment for disruptive or unsafe behavior without refund.

LIABILITY, INDEMNIFICATION & ASSUMPTION OF RISK

Student (and Parent/Guardian on behalf of a minor Student) acknowledges that participation in instruction involves inherent risks. To the fullest extent permitted by applicable law, Student releases, waives and discharges Provider, its instructors, employees and agents from liability for injury, loss or damage arising out of or related to participation, except where such injury results from Provider's gross negligence or willful misconduct. Student agrees to indemnify and hold harmless Provider from any claims, causes of action, damages, judgments, costs or expenses arising from Student's acts or omissions.

PHOTO & MEDIA RELEASE

I consent to Provider's use of photographs, recordings or other media featuring the Student for educational, promotional, or instructional purposes, provided that no personally identifiable information will be published without separate written consent.

CONFIDENTIALITY & INTELLECTUAL PROPERTY

Instructional materials provided by Provider are proprietary. Student agrees not to reproduce, distribute or publish such materials without Provider's prior written consent. Provider retains ownership of original curricula and pedagogical materials; Student is granted a limited license for personal educational use only.

TERMINATION, DEFAULT & REMEDIES

Provider may terminate this Contract for material breach, including non-payment or disruptive conduct, after providing written notice and a reasonable opportunity to cure where practicable. In the event of Student default, Provider may retain amounts paid and seek collection of unpaid tuition, together with reasonable collection costs and attorney fees to the extent permitted by law.

GOVERNING LAW & DISPUTE RESOLUTION

This Contract is governed by the laws of the state in which Provider maintains its primary place of business, without regard to conflicts of law principles. Parties agree to attempt good-faith negotiation of disputes and, if unresolved, to submit disputes to binding arbitration in the Provider's county of primary business, unless prohibited by applicable law.
