Whereas, Complainant, a single woman, gave birth to a _____________________ (male or female) child on ______________________________________ (date), at _______________ ___________________________________________ (name of hospital), __________________ _________________________________________________________________________________________ (street address, city, county, state, zip code) ; and Whereas, Complainant has initiated proceedings against Defendant in the _______________________ (name of court) Court of _________________________ (name of county) County, _____________________________ (name of state) , Cause No. __________________, alleging that Defendant is the father of her child and seeking an order for support and maintenance of the child; and Whereas, Defendant denies that he is the father of the child, but desires nevertheless to effect a compromise in settlement of the proceedings against him and to be released from all other claims with respect to the child; Now, therefore, for and in consideration of the mutual covenants contained in this agreement, and other good and valuable consideration, the receipt and sufficiency of which is hereby acknowledged, the parties agree as follows: 1. Complainant shall abandon the above-mentioned proceedings against Defendant. Complainant hereby releases Defendant from all liability with respect to any and all claims she now has, or may later have, pertaining to the paternity, support, or maintenance of the child.2. Contemporaneously with the execution of this Agreement, the parties shall execute a support agreement by the terms of which Defendant shall reimburse Complainant for expenses of pregnancy and birth of the child, and shall assume liability for making payments to Complainant for the support of the child in the amounts, and under the conditions, set forth in such support agreement. A copy of the support agreement is attached to and made a part of this Agreement as Exhibit A.
