Student agrees to attend scheduled sessions punctually and to adhere to instructor directions and the program code of conduct. Excessive unexcused absences may result in removal from the program without refund as specified in the refund policy below.

Cancellation and Refund Policy

All cancellations must be submitted in writing to the Program Provider. Deposits are non-refundable except where otherwise expressly stated. If withdrawal occurs more than 30 days prior to program start, tuition paid beyond the deposit will be refunded minus administrative processing fee. If withdrawal occurs within 30 days of start or after commencement of services, no refund will be provided except at the sole discretion of the Program Provider. Refunds, when approved, will be calculated on a pro-rata basis for sessions not yet provided, less any non-refundable fees and costs incurred.

Liability Release and Consent

By signing below, the undersigned releases the Program Provider, its instructors, agents and employees from liability for personal injury and loss of property arising from participation in the program, except where such injury or loss is caused by the gross negligence or intentional misconduct of the Program Provider. The undersigned consents to emergency medical treatment deemed necessary by authorized medical personnel in the event of injury or illness and agrees to be responsible for medical expenses incurred.

Privacy, Records and Intellectual Property

The Program Provider will maintain records necessary for program administration. Student work created during the program may be used by the Program Provider for promotional or instructional purposes unless the undersigned notifies the Program Provider in writing to restrict use. The Program Provider retains no obligation to preserve student materials beyond standard retention periods.

The undersigned certifies that the information provided in this Contract is true and complete to the best of their knowledge. The undersigned further certifies that they have the authority to enter into this Contract (or, if signing as a parent/guardian, that they have authority to enroll the minor student) and that they agree to abide by the terms and policies set forth herein.
