Authorization for Emergency Medical Treatment: I authorize program staff and affiliated medical personnel to obtain and provide emergency medical treatment for the student when, in the judgment of program officials, such treatment is necessary.

Participation Waiver & Release of Liability

Acknowledgment of Risk: I understand and acknowledge that participation in soccer and related activities involves inherent risks including but not limited to sprains, fractures, concussions, and other injuries which may result in serious bodily injury or death. I voluntarily assume all risks associated with participation.

Release and Indemnity: In consideration of the student being permitted to participate in the program, I, on behalf of the participant, parent(s), guardian(s), heirs, personal representatives and assigns, hereby release, waive, discharge and covenant not to sue the club, its governing body, coaches, volunteers, sponsors, agents, employees, and affiliates (collectively, the Program) from liability for any and all claims, demands, actions or causes of action, whether known or unknown, arising out of or related to any loss, damage or injury, including death, that may be sustained as a result of participation, except in cases of willful misconduct or gross negligence proven by the claimant.

Indemnification: I agree to indemnify and hold harmless the Program from any loss, liability, damage or cost, including reasonable attorneys' fees, that arise out of or are related to the participant's actions or omissions.

Concussion Notice: I acknowledge receipt of information regarding the signs, symptoms and risks of concussion and head injury. I understand that I must remove the participant from play if concussion is suspected and follow return-to-play protocols.

Photo & Media Release

I authorize the Program to take and use photographs, video, and/or audio recordings of the participant for promotional, educational, or training purposes. I consent I do not consent

Code of Conduct & Discipline

The participant and parent/guardian agree to abide by the Program's code of conduct, including respectful behavior toward coaches, officials, participants and spectators, and adherence to team rules. Failure to comply may result in suspension or removal from the program without refund.
