Fees, Payment, and Refunds

Late payments incur a fee of per missed payment and interest at the lesser of 1.5% per month or the maximum permitted by law. Cancellation by Participant with written notice received at least days prior to the next scheduled session will result in a refund of prepaid amounts less a reasonable administrative fee as set by Provider.

Attendance, Make-up Sessions, and Conduct

Participant agrees to attend scheduled sessions punctually. Provider's make-up policy: make-up sessions will be offered only when Provider cancels a session or when Participant provides at least 24 hours' notice of absence, subject to Provider's reasonable scheduling. Repeated unexcused absences may result in termination of services.

Medical Information, Emergency Authorization & Assumption of Risk

Participant and Parent/Guardian hereby acknowledge that participation in athletic training involves inherent risks including physical injury. Participant assumes all risks and releases and holds harmless Provider, its employees, agents, and affiliates from claims arising out of participation, except for gross negligence or willful misconduct by Provider. In the event of an emergency, Provider is authorized to seek medical treatment for Participant and to arrange transportation to an appropriate facility when necessary; costs for such treatment are the responsibility of Participant or Parent/Guardian.

Media Release and Data Use

Participant grants Provider a non-exclusive, royalty-free license to use photographs, video, and performance data collected in connection with training for promotional, educational, or analytical purposes, unless Participant or Parent/Guardian opts out by checking the box below.

Training programs, drills, written materials, and training plans are proprietary to Provider. Participant shall not copy, broadcast, or commercially use Provider's materials without prior written consent. Termination, Remedies and Governing Law

Either party may terminate this Contract for material breach after providing written notice and a reasonable opportunity to cure. Provider may immediately terminate services for safety or conduct violations. Disputes arising under this Contract shall be governed by the laws of the State of . Parties agree to attempt good faith resolution prior to pursuing formal legal action.

Additional Terms and Acknowledgments

No guarantee of specific athletic outcomes, selection, or scholarship is made by Provider.
