Refund Policy: If written cancellation is received by the Provider at least days prior to the session start date, Provider will refund tuition less the non-refundable deposit. Withdrawals after that time are subject to a prorated refund at Provider's discretion and may be reduced for costs already incurred. No refunds will be issued for participant dismissal for cause.

PROGRAM RULES, BEHAVIOR AND DISCIPLINE

Participant is expected to follow all program rules and staff instructions. Provider reserves the right to suspend or dismiss any participant whose behavior endangers the participant or others, or materially interferes with program operation. In the event of dismissal for cause, no refund will be provided and Parent/Guardian is responsible for immediate removal of the participant.

MEDICAL AUTHORIZATION AND LIABILITY RELEASE

Parent/Guardian hereby authorizes Provider and its staff and medical personnel to administer or arrange emergency medical treatment to the participant if, in the judgment of Provider, such treatment is necessary. Parent/Guardian authorizes release of medical information to emergency personnel as necessary. Parent/Guardian understands and agrees that Provider and its officers, directors, employees, volunteers and agents shall not be liable for injuries resulting from ordinary negligence, and Parent/Guardian releases and agrees to indemnify and hold harmless Provider from claims arising out of participation except to the extent such claims arise from gross negligence or willful misconduct.

I authorize Provider to obtain necessary medical treatment for the participant in an emergency and agree to be responsible for all associated costs. I agree Provider may transport the participant to obtain emergency medical care if necessary.

PHOTO, VIDEO AND MEDIA RELEASE

I consent to the use of photographs, video and digital images of the participant for promotional, educational, or archival purposes by Provider without further compensation. I understand that Provider will not identify participants in media without separate consent unless required for safety or emergency purposes.

TRANSPORTATION AND FIELD TRIP AUTHORIZATION

I authorize Participant to be transported by Provider-authorized vehicles and chaperones for program activities and field trips that are part of the session. Provider will provide supervision in accordance with Provider policies.

ACKNOWLEDGMENTS, WARRANTIES AND AGREEMENT

Parent/Guardian warrants that the information provided in this Contract is true and complete. Parent/Guardian agrees to notify Provider in writing of any changes to health, emergency contact or other material information prior to the start of the session. This Contract constitutes the entire agreement between the parties with respect to enrollment and supersedes prior discussions. Any amendment must be in writing and signed by an authorized representative of Provider.
