STAFF ENROLLMENT CONTRACT

This Staff Enrollment Contract is made effective as of by and between Institution Name: (hereinafter referred to as "Institution") and Staff Member: (hereinafter referred to as "Enrollee"). The parties agree as follows:

If the Enrollee withdraws prior to the program start date, the Institution will refund of tuition, less any non‑refundable administrative fees. If withdrawal occurs after the program start date, the Enrollee agrees to reimburse the Institution of tuition, unless otherwise expressly agreed in writing.

The Enrollee must comply with the program attendance and assessment requirements established by the Institution. Failure to attend, complete required assessments, or meet minimum performance standards may result in termination of enrollment and an obligation to reimburse the Institution pursuant to the repayment provisions above.

4. Intellectual Property; Confidentiality

All materials provided by the Institution remain the property of the Institution unless otherwise agreed in writing. The Enrollee shall not reproduce, distribute, or disclose confidential materials without prior written consent. Any course materials developed jointly shall be addressed under the Institution's intellectual property policy; absent agreement, ownership vests with the Institution.

5. Data, Records and Privacy

The Institution will collect and retain records necessary to administer the program. The Enrollee consents to the collection and use of personally identifiable information for program administration, certification, and billing purposes in accordance with institutional policies.

6. Termination and Remedies

Institution may terminate this Contract for cause if the Enrollee breaches institutional policies or fails to meet program requirements. The Institution may also terminate for administrative reasons with reasonable notice. In the event of termination, the parties' financial obligations will be determined consistent with the refund and repayment provisions set forth herein.

The Enrollee agrees to indemnify and hold harmless the Institution from claims arising from the Enrollee's negligent acts or willful misconduct in connection with program participation.

The Enrollee represents that all information provided to the Institution in connection with this enrollment is true and complete. The Enrollee will comply with all applicable institutional policies, codes of conduct, and any accreditation or professional standards relevant to the program.
