Acceptance: By signing below, the undersigned accepts the offer of admission to the program listed above and agrees to comply with all published academic and conduct standards of the institution. Acceptance is effective upon receipt of any required deposit and completion of this signed agreement.

Financial Responsibility: The student or guarantor is financially responsible for all tuition, fees, and charges assessed to the student account. All fees are due in accordance with the payment arrangement selected. Late payments may incur finance charges or late fees and may result in denial of access to classes, transcripts, or credential conferral.

Deposit and Refunds: Enrollment deposit is applied to tuition. Unless otherwise specified in writing, deposit is non-refundable after the specified deadline. Refunds for withdrawal follow the institution's published refund schedule. The institution reserves the right to withhold refund for outstanding account balances or failure to complete required withdrawal steps.

Withdrawal and Dismissal: Withdrawal must be submitted in writing. The institution may dismiss a student for failure to meet academic standards, breaches of the student code of conduct, non-payment, or other cause. Dismissal does not relieve the student or guarantor of financial obligations incurred through the date of dismissal.

Records and Privacy: The institution maintains student records and will release or disclose information only as permitted by institutional policy and applicable law. The undersigned authorizes the institution to share enrollment status and billing information with the listed guarantor or parent/guardian when necessary for administration of this agreement.

Code of Conduct and Academic Policies: The student affirms that they have read, or will read prior to attendance, the institution's code of conduct, attendance policy, academic integrity policy, and other academic requirements and accepts these as conditions of enrollment.

Medical Authorization: In the event of an emergency, I authorize the institution to seek and consent to emergency medical treatment for the student if I cannot be reached. I understand that I am responsible for medical expenses not covered by insurance.
