Authorizations, Assumption of Risk, and Release

I, the undersigned, am the parent/legal guardian of the named student or the student if over 18, and I authorize the student's participation in the above-described activity. I understand participation may involve travel and physical activity and I accept responsibility for assessing the student’s ability to participate.

Assumption of Risk: I acknowledge that there are inherent risks associated with participation in the activity, including the risk of bodily injury. I knowingly and voluntarily assume all risks associated with participation and transportation to and from the activity.

Release and Indemnification: To the fullest extent permitted by law, I release and hold harmless the school district, its employees, volunteers, agents, and representatives from any and all liability for personal injury, property damage, or other loss arising from or in connection with the student's participation, except to the extent caused by gross negligence or willful misconduct. I agree to indemnify and defend the released parties against claims arising from my child's actions.

Emergency Treatment Authorization: If medical treatment is necessary and I cannot be reached, I authorize school personnel or an authorized adult to obtain emergency medical treatment for the student and to consent to medical and surgical procedures as may be necessary in the judgment of the attending medical personnel.
