Privacy and records: the undersigned authorizes the release and exchange of academic and health records between prior institutions and this school as reasonably necessary for enrollment and placement. Educational records will be maintained and disclosed in accordance with applicable law; directory information may be used for school publications unless the parent/guardian opts out below.

Media release: I grant permission for the student to be photographed, videotaped, or recorded for educational, informational, or promotional purposes and for such media to be used by the school unless I expressly withhold consent below.

Field trip and activity consent: I authorize the student to participate in school-sponsored activities and field trips. I understand that additional notices will be provided for off-site or overnight events and that separate consents may be required for certain activities.
