The Host Employer agrees to maintain appropriate liability and workers' compensation insurance covering the apprentice as required by applicable law and to provide evidence of such coverage on request. The Program Provider will maintain general liability coverage for on-campus activities.

TERMINATION & WITHDRAWAL

Either party may terminate this Agreement for cause upon written notice specifying the reasons. The Host Employer or Program Provider may suspend or terminate participation for breaches of safety rules, unlawful conduct, or failure to meet academic requirements. Apprentice may withdraw with at least days' prior written notice, subject to program requirements.

CONFIDENTIALITY, INTELLECTUAL PROPERTY & DATA

Apprentice agrees to maintain the confidentiality of proprietary information obtained during the apprenticeship. Work product created by the Apprentice in the scope of the apprenticeship shall be handled in accordance with the Host Employer's written intellectual property policy. Where no separate policy exists, the Host Employer retains ownership of work product created in connection with business operations. The Program Provider may use de-identified assessment data for program evaluation.

ACKNOWLEDGMENTS & CERTIFICATIONS

By signing below, the Apprentice (or Parent/Guardian if the Apprentice is a minor) certifies that the information provided herein is true and complete to the best of their knowledge, that they consent to program participation, and that they understand the rules, responsibilities, and risks described in this Agreement. The Program Provider and Host Employer certify that they will provide the agreed training, supervision, and safety protections consistent with applicable law.

Indemnification: The Apprentice (or Apprentice's Parent/Guardian) agrees to indemnify and hold harmless the Program Provider from claims arising from willful misconduct or material breach of this Agreement by the Apprentice. The Host Employer agrees to indemnify the Program Provider for claims arising from the Host Employer's negligence in training or supervision.

ADDITIONAL PROVISIONS

Governing Law: This Agreement shall be governed by and construed in accordance with the laws of the state in which the primary training location is located, without regard to conflict of law principles.
