In the event of medical emergency the undersigned authorizes Program Provider and its representatives to obtain emergency medical care for the student. The undersigned shall be responsible for all costs associated with such care. By initialing below the undersigned grants emergency treatment authorization:

Assumption of Risk; Liability Waiver; Indemnification

The undersigned acknowledges that participation in athletic training and related activities involves physical contact and risk of injury. The student and guardian assume all risks associated with participation. The undersigned releases and forever discharges the Program Provider, its officers, employees, agents and contractors from any and all claims, demands, causes of action and liabilities arising from participation, except for willful misconduct or gross negligence. The undersigned agrees to indemnify and hold harmless the Program Provider from any third-party claim arising from the student’s acts or omissions.

I grant permission for photographs, video, and performance data of the student to be used by the Program Provider for instructional, promotional, and evaluation purposes. No compensation is due for use. If consent is not granted, do not select the checkbox below.

Code of Conduct & Attendance

Participants are expected to maintain respectful conduct, follow staff directions, attend scheduled sessions punctually, and comply with any facility rules. Repeated violation of the code may result in suspension or termination of services without refund.

Cancellation & Refund Policy

Cancellations must be submitted in writing. Fees prepaid for future sessions are refundable only as set forth in the provider's cancellation policy; administrative fees may apply. No refunds will be issued for sessions already conducted. The Program Provider may cancel or reschedule sessions due to weather, facility closure, or instructor availability; in such cases, make-up sessions or prorated refunds will be provided at the provider's discretion.

Data Use & Privacy

The Program Provider will collect and retain contact, medical and performance information necessary to deliver services. Such data will be stored securely and used for program administration, training plans, and health and safety. The provider will not disclose sensitive medical data except as required for emergency treatment or as authorized by the undersigned.
