By signing below, the signer authorizes the Institution named above to initiate electronic debits to the bank account or credit/debit card provided for the described tuition and fees. This authorization remains in effect until the Institution receives written revocation signed by the account holder and has had a reasonable opportunity to act (not to exceed 30 days from receipt). The signer certifies that the account information provided is correct and that they are an authorized signer on the account.

The Institution may charge previously scheduled payments in accordance with the payment schedule. If a payment is returned unpaid, the Institution may attempt re-presentment and may assess a returned payment fee in accordance with institutional policy. The signer is responsible for any fees assessed by the financial institution resulting from the authorization.

The Institution will provide notice of any change in the amount or timing of scheduled debits at least 10 days prior to the change. The signer understands that cancelling or changing this authorization requires written notice to the Institution and does not affect debits already processed in accordance with this authorization.

The signer agrees to hold the Institution harmless from any claim, loss or liability resulting from the Institution's reliance on the information provided, except for liability resulting from the Institution's gross negligence or willful misconduct. This agreement is governed by the laws applicable to the Institution's principal place of business.
