Terms, Conditions and Requirements

1. Enrollment Requirement. Student must be enrolled and actively attending the Institution as a student during the award period. Failure to maintain the required enrollment status may result in suspension or forfeiture of the Award.

2. Minimum Academic Standard. Student must maintain a cumulative grade point average (GPA) of at least . The Institution will review academic standing at the end of each term; failure to meet the minimum GPA will result in probation or termination of funding as described below.

3. Use of Award. Award funds are intended for eligible educational expenses such as tuition, fees, required course materials, and authorized campus housing when applicable. Use for other purposes is prohibited unless expressly approved in writing by the Institution.

4. Verification and Reporting. Student agrees to provide official grade reports, enrollment verification, and any other documentation reasonably requested by the Institution to determine continued eligibility. Failure to provide requested documentation within days of request may result in withholding of disbursements.

5. Forfeiture and Repayment. If Student is found ineligible, withdraws, is dismissed for disciplinary reasons, or otherwise fails to satisfy the conditions of this Agreement, the Institution may (a) suspend future payments, (b) demand repayment of funds already disbursed on a pro rata basis, and/or (c) offset future institutional credits. Repayment obligations shall be calculated in accordance with the Institution's written policy and are due within days of notice.

6. Concurrent Awards. Student must promptly disclose any other scholarships, grants, or awards received. The Institution reserves the right to adjust the Award to prevent duplication of funding for the same expenses.

Privacy, Publicity and Tax Treatment

7. Publicity. By accepting the Award, Student grants the Institution the right to use Student's name, degree program, and award citation for non-commercial recognition and publicity purposes unless Student indicates objection below.

8. Tax Liability. Student acknowledges that portions of the Award may be taxable under applicable law. Student is solely responsible for determining and satisfying any tax obligations arising from receipt of the Award.

9. Modification and Termination. This Agreement may be modified only by a written amendment signed by both parties. The Institution may terminate the Award for cause upon written notice to Student.

10. Governing Law. This Agreement will be governed by and construed in accordance with the laws of the jurisdiction where the Institution is located:
