Consent for Emergency Treatment & Release of Liability

I, the undersigned parent or legal guardian (or adult student if aged 18 or older), authorize program staff to obtain and approve emergency medical care for the student named above if the parent/guardian cannot be reached. In the event of emergency, I authorize medical providers to administer treatment deemed necessary for stabilization and ongoing care. I agree to be responsible for costs of such care.

I acknowledge that backpacking and related activities involve inherent risks including, but not limited to, uneven terrain, weather exposure, wildlife encounters, water crossings, altitude effects, and the risk of injury or death. I voluntarily assume all such risks and agree that the program, its employees, volunteers, and affiliated organizations shall not be liable for any injury, loss, or damage except for gross negligence or willful misconduct. I further agree to indemnify and hold harmless the program from any claims arising out of the student's participation except those arising from the program's gross negligence or willful misconduct.

Photo / Video Release

I consent to the use of photographs and video recordings that may include the student for program promotional, educational, or informational purposes. I understand no identifying personal contact information will be published without separate consent.

Transportation & Pickup Authorization

I authorize transportation of the student by program-owned or contracted vehicles for the duration of the program and related activities. I authorize release of the student only to the parent/guardian or other persons listed below at the conclusion of the trip.

Physical Fitness Acknowledgment

I affirm that the student is physically capable of participating in backpacking activities and has trained sufficiently for the anticipated distance and elevation of the trip. If I have concerns, I have discussed them with the program prior to the trip.
