Assumption of Risk, Release, and Indemnification

I, the undersigned, acknowledge that participation in baseball activities exposes the participant to inherent risks including, but not limited to, bodily injury, concussion, paralysis, or death, and damage to personal property. I understand that such risks cannot be eliminated without jeopardizing the essential qualities of the activity.

In consideration for allowing the student to participate, I expressly assume all risk of injury or harm and hereby release, waive, discharge and covenant not to sue the School, the School District, the Board of Education, coaches, trainers, officials, employees, volunteers, agents, and representatives (collectively, Released Parties) from any and all liability, claims, demands, actions or causes of action arising out of or related to any loss, damage or injury, including death, that may be sustained while participating in baseball activities, whether caused by the negligence of the Released Parties or otherwise, to the fullest extent permitted by law.

I agree to indemnify, defend, and hold harmless the Released Parties from and against any and all claims, liabilities, costs and expenses (including reasonable attorneys' fees) arising out of or connected with the student's participation in baseball activities, except to the extent such claims result from the gross negligence or willful misconduct of a Released Party.

In the event of injury or illness, if emergency medical treatment is necessary and I cannot be reached, I authorize coaches, trainers, or school personnel to secure emergency medical care and transportation as they deem necessary. I understand I am responsible for all costs associated with such care and transportation.

I authorize transportation by school-approved vehicles, volunteers, or designated drivers for games and sanctioned activities.

Media Release & Conduct

I grant permission for the student's photograph, image or video to be used by the School for promotional, educational, or informational purposes in print or electronic media unless I opt out below.
