The participant and parent/guardian voluntarily assume all risks associated with participation. The participant and parent/guardian release, waive, discharge and covenant not to sue the organization, its officers, employees, coaches, volunteers, and agents for any claim arising out of injury, death, or property loss, whether caused by negligence or otherwise, to the fullest extent permitted by law.

Emergency Medical Authorization: If emergency medical treatment is necessary and the parent/guardian cannot be reached, I authorize coaches or other authorized representatives to obtain and consent to medical treatment for the participant. I agree to be responsible for all costs related to such treatment.

Indemnification: The participant and parent/guardian agree to indemnify and hold harmless the organization and its agents from any claims, damages, losses or expenses, including reasonable attorneys' fees, arising out of participation in the program.
