Cancellation and Rescheduling: Bookings cancelled fewer than 72 hours prior to the scheduled start time are subject to a cancellation fee equal to 50% of the scheduled fee. Cancellations within 24 hours may forfeit the entire fee. Rescheduling requests are accepted subject to staff and space availability. By signing this form the signing party accepts these terms.

Liability and Indemnification: The signing party acknowledges that while the institution will take reasonable precautions to ensure a safe environment, the institution is not liable for loss or damage to personal property and is not responsible for injuries resulting from the signing party's failure to follow facility rules. The signing party agrees to indemnify and hold harmless the institution, its employees, and agents from claims arising out of the signing party's acts or omissions during the booked activity, except to the extent caused by the institution's gross negligence or willful misconduct.
