I, the undersigned, acknowledge that participation in the buddy program described above may involve routine physical activity, movement about school property or other sites, and interaction with other students. I understand and accept that such activity carries certain inherent risks, including but not limited to minor injury or illness. I voluntarily assume all risks associated with participation by the student named above.

In consideration for permitting the student to participate, I hereby release, waive, discharge, and covenant not to sue the School, its officers, trustees, employees, agents, volunteers, and program supervisors from any and all liability, claims, demands, actions, and causes of action whatsoever arising out of or related to any loss, damage, or injury, including death, that may be sustained by the student while participating in the buddy program or while on the premises where the program is conducted, except for claims resulting from the School’s gross negligence or willful misconduct.

I further agree to indemnify and hold harmless the School and its affiliates against any claims, damages, losses, liabilities, and expenses (including reasonable attorney fees) arising from the student’s participation, whether caused by the student’s actions or the actions of others, except to the extent that such claims arise from gross negligence or willful misconduct of the School.
