Medical Authorization:
I hereby authorize camp staff to obtain emergency medical care for my child if necessary and to consent to examination, diagnosis, and treatment by licensed medical personnel when I cannot be reached. I understand I will be notified as soon as practicable.

Assumption of Risk & Release

Assumption of Risk and Release of Liability: Participation in camp activities involves inherent risks which may include physical injury. On behalf of myself and the student named above, I expressly assume all risks associated with participation, and I release, waive and covenant not to sue the camp, its directors, employees, volunteers and agents for any liability for personal injury, property damage, wrongful death, or other loss arising out of participation, except to the extent caused by the camp's gross negligence or willful misconduct.

Cancellation, Refunds & Financial Responsibility

Payment Terms and Refunds: All fees are due per the chosen payment plan. Cancellations received in writing prior to the program start may be subject to administrative fees. No refunds will be issued for dismissal due to misconduct. The parent/guardian is responsible for all unpaid balances and for any costs incurred in collection of unpaid balances.
