STUDENT CAMP POLICIES

This document sets forth the policies, consents, authorizations, and acknowledgments required for participation in the camp program. Completion and signature indicate that the signer has read, understands, and agrees to comply with the policies listed below and authorizes the camp to take actions as specified in the medical and emergency provisions.

I authorize camp staff to administer the medications listed above in accordance with the provided instructions and to communicate with the prescribing physician or pharmacist as necessary for safe administration.

Policies, Conduct & Safety

The camp maintains codes of conduct to protect the safety and welfare of participants. Prohibited conduct includes any activity that endangers self or others, possession of weapons, use or distribution of illegal substances, bullying, or persistent refusal to follow staff directives. Staff may impose disciplinary measures including time-out, restricted participation, or dismissal from the program without refund for serious or repeated violations.

I have reviewed the student code of conduct and understand the disciplinary procedures and grounds for dismissal. I acknowledge that dismissal for cause may occur and that dismissal does not relieve me of financial obligations incurred prior to dismissal.

Transportation Consent

Camp activities may include supervised local transportation by camp vehicles or contracted carriers. Transportation will be conducted in compliance with applicable safety standards.

I consent to my child's participation in camp-sponsored transportation for planned activities and excursions.

Media Release

Photographs, video, or audio recordings may be taken of camp activities for use in program materials, promotional items, or educational purposes. Recordings will not be used for commercial sale.

I grant permission for the camp to photograph and/or record my child and to use such media for program-related purposes.    I do NOT grant permission for media use.

Cancellation, Fees & Refunds

Registration fees, camp tuition, deposit amounts, cancellation deadlines, and refund schedules are governed by camp registration policies. Fees paid for enrollment may be non-refundable after specified deadlines. Financial obligations remain due for services rendered prior to cancellation or dismissal.

Assumption of Risk, Release and Indemnification

Participation in camp activities involves inherent risks, including physical injury. By signing this form, the signer voluntarily assumes the risks associated with participation. To the fullest extent permitted by law, the signer releases, waives, and discharges the camp, its governing body, employees, volunteers, agents, and affiliated organizations from liability for ordinary negligence and agrees to indemnify and hold harmless the released parties from claims, costs, and damages arising out of participation, except for claims arising from gross negligence or willful misconduct.

Emergency Medical Consent

If an emergency arises and the parent/guardian cannot be reached, the signer authorizes camp staff to obtain necessary medical treatment and transport the student to a medical facility. The signer agrees to be financially responsible for such treatment and accepts that reasonable attempts will be made to notify the emergency contact.
