Medical Release: By signing this Agreement, the undersigned authorizes the Educational Provider to obtain emergency medical care for the Student if a parent or guardian cannot be reached. The undersigned understands that the Educational Provider is not liable for costs of medical care; the undersigned agrees to be responsible for such costs.

Media, Technology & Privacy

Photo/Video Release: I consent to the Educational Provider's use of photographs and recordings of the Student for educational, promotional, and archival purposes, subject to any limitations specified in writing.

Technology and Conduct: The Student agrees to comply with the Educational Provider's acceptable use policies, to respect privacy and intellectual property, and not to record or distribute classroom materials without prior written permission.

Cancellation, Refunds & Termination

Cancellation and Refund Policy: Refunds, prorated tuition, and forfeiture are determined according to the Educational Provider's policy. By signing, the undersigned acknowledges the refund schedule and understands that early withdrawal or dismissal may not entitle the Student to a full refund.

Termination for Cause: The Educational Provider may suspend or terminate the Student's participation for violation of policies, misconduct, or failure to comply with program requirements. No termination relieves the undersigned of outstanding financial obligations incurred prior to termination.

Acknowledgements & Certifications

By checking each box and signing below, the undersigned certifies that they have read, understand, and agree to the terms of this Agreement, the applicable policies, and the obligations set forth herein.

Governing Law and Venue: This Agreement is governed by the laws of the state or jurisdiction where the Educational Provider operates. Any dispute arising under this Agreement will be resolved in the appropriate courts located in that jurisdiction, unless otherwise agreed in writing.
