Services and Responsibilities

The Coach agrees to provide individualized and/or group coaching services for the Student in the subject matter identified above. Services will include lesson planning, skills instruction, progress assessment, and periodic reporting to the Student or Parent/Guardian. The Coach shall perform services in a professional manner consistent with industry standards and applicable school or program policies.

Student / Parent Obligations

The Student and Parent/Guardian agree that the Student will attend scheduled sessions, arrive prepared with necessary materials, comply with all safety and conduct requirements, and participate in assigned practice or assignments. Repeated absence or failure to comply may result in suspension or termination of services.

Fees, Payment, Cancellation and Refunds

Cancellation and Refund Policy: Fees are refundable only as stated here. Partial refunds require written notice at least days prior to the start date. No refunds for sessions already rendered. Coach reserves the right to terminate for cause without refund for policy breaches.

Confidentiality and Records

All educational records and progress reports prepared by the Coach are the property of the program unless otherwise agreed in writing. The Coach will maintain confidentiality of student records and personal information except as required by law or where disclosure is necessary to protect the health and safety of the Student or others.

Liability, Release and Indemnification

To the fullest extent permitted by law, the Student and Parent/Guardian release and hold harmless the Coach, its employees and agents from liability for personal injury, property damage, or other loss arising out of the provision of coaching services, except where caused by the Coach's gross negligence or willful misconduct. The Student and Parent/Guardian agree to indemnify and defend the Coach against claims arising from the Student's actions or failure to follow safety instructions.

Termination

Either party may terminate this Agreement for material breach upon written notice. The Coach may suspend or terminate services immediately for safety concerns, nonpayment, or serious misconduct. Upon termination the Student remains responsible for fees for services already provided and for any unpaid amounts accrued under agreed payment schedules.

Medical and Emergency Information

Parent/Guardian authorizes the Coach to obtain emergency medical treatment for the Student when immediate treatment is necessary and the Parent/Guardian cannot be reached.

Photo/Video Release:   I consent to the use of photographs or video of the Student for program materials and promotional purposes, subject to the confidentiality provisions above.

Acknowledgment and Certification
