Confidentiality & Records

All records and information exchanged in the coaching relationship are treated as confidential and will not be released without written authorization, except as required by law, including obligations to report harm, or as necessary to respond to legal process. Progress notes maintained by the Coach remain the property of the Coach unless otherwise agreed in writing. Requests for release of records must be submitted in writing and will be honored in accordance with applicable legal requirements.

I authorize the Coach to communicate directly with school personnel regarding goals, accommodations, and progress as necessary to support implementation of the coaching plan.

Acknowledgement, Responsibilities & Termination

The Student and Parent/Guardian (if Student is a minor) acknowledge that coaching is a collaborative process requiring active participation. The Student/Parent/Guardian agrees to attend scheduled sessions, complete assigned tasks between sessions, and communicate concerns promptly. The Coach agrees to provide services consistent with professional standards. Either party may terminate services with notice; fees for services already delivered are non-refundable unless otherwise specified in writing.

In the event of an emergency, I authorize the Coach to obtain or assist in obtaining emergency medical treatment for the Student when a Parent/Guardian cannot be reached.
