Cancellation and Refund Policy: The Provider requires written notice for cancellation. Refunds, if any, will be calculated pro rata for unused pre-paid sessions, less any administrative fees. Any refund request must be submitted to the Provider in writing and will be processed in accordance with this Agreement. The Provider reserves the right to terminate services for breach of the Agreement without refund where the Participant fails to comply with program rules or payment obligations.

Responsibilities, Conduct, and Academic Policies

Participant Responsibilities: The Participant agrees to attend scheduled sessions punctually, complete assigned work between sessions, notify the Provider of absences at least 24 hours in advance where possible, and conduct themselves respectfully. The Provider will make reasonable efforts to provide the coaching services described, but cannot guarantee academic outcomes.

Code of Conduct: Abusive, discriminatory, or harassing behavior toward Provider staff or other participants constitutes material breach and may result in immediate termination of services without refund. The Provider retains discretion to modify session format or scheduling to maintain a safe and effective coaching environment.

Confidentiality, Records, and Information Sharing

Confidentiality: Except as required by law or as necessary for the safety of the Participant, the Provider will maintain confidential records of coaching sessions and Student materials. The Provider may share relevant information with the Participant's school, teachers, or designated guardians only with prior written consent as indicated below.

Medical & Emergency Information

Medical Authorization: In the event of emergency where the guardian cannot be reached, I authorize the Provider to seek emergency medical treatment for the Student and to share necessary medical information with emergency personnel.

Liability, Indemnity, and Limitation of Liability

Liability Waiver: The Participant agrees that the Provider, its employees, contractors, and agents will not be liable for indirect, incidental, special, or consequential damages arising out of or related to the coaching services, including but not limited to any academic outcome. To the fullest extent permitted by law, the Participant releases and indemnifies the Provider from any claims arising from participation in coaching services except for gross negligence or willful misconduct by the Provider.

Data Protection

The Provider will retain Participant records in accordance with its records retention policy. Participant consents to the collection and processing of personal data necessary to provide coaching services, billing, and program administration.

Acknowledgment and Certification

By signing below the Participant certifies that the information provided in this Agreement is true and correct, that they have the authority to enroll the Student (or are the Student), and that they have read and understood all terms, policies, and acknowledgements contained herein.
