Assumption of Risk, Release and Indemnity

In exchange for the student’s participation and for valuable consideration, the undersigned, on behalf of the student and the undersigned's heirs, assigns and personal representatives, acknowledges and agrees that participation may involve certain risks, including but not limited to physical injury, illness, or property damage. The undersigned voluntarily assumes all risks associated with participation, whether known or unknown, and including those arising from the negligence of the school, its employees, volunteers, contractors, or agents.

The undersigned hereby releases, waives, discharges and covenants not to sue the school, its governing board, officers, employees, volunteers and agents (collectively the Released Parties) from any and all liability, claims, demands, actions or causes of action, known or unknown, arising out of or related to any loss, damage or injury sustained by the student during participation, including claims based on the negligence of the Released Parties.

The undersigned further agrees to indemnify, defend and hold harmless the Released Parties from and against any and all claims, liabilities, losses, damages and expenses (including reasonable attorneys' fees) arising from the student's actions or omissions during the activity, or from a breach of any representation or agreement made in this form.

Authorization for Medical Treatment

If the undersigned cannot be reached in an emergency, the undersigned authorizes school officials or designated agents to obtain any medical care deemed necessary for the student's welfare, including emergency transportation, hospitalization, anesthesia, or surgery. The undersigned agrees to be financially responsible for such treatment and releases the Released Parties from any liability for their efforts to obtain medical care.

Representations, Acknowledgement and Governing Terms

The undersigned represents that all information provided on this form is true and complete to the best of their knowledge. The undersigned acknowledges receipt of applicable program rules and agrees that the student will comply with all directions, policies and safety protocols. If any provision of this agreement is held unenforceable, the remaining provisions shall remain in full force and effect. This document constitutes the entire agreement between the parties regarding the subject matter herein.
