Supervision: The student will be under the supervision of designated staff and volunteers from the sponsoring organization for the duration of the convention, including scheduled travel periods. By signing below, the undersigned acknowledges and accepts the supervisory arrangements and responsibilities described herein.

Payment Terms: Payment of fees is required by the deadlines set by the sponsoring entity. Failure to remit required fees may result in cancellation of participation. Refund and cancellation policies are governed by the sponsor and will be applied as stated in the sponsor's published policy.

Code of Conduct & Permissions

The student agrees to abide by all rules, schedules, and directions of supervisors. The undersigned acknowledges that failure to comply with the code of conduct may result in disciplinary action, including removal from convention activities and immediate return home at the expense of the parent/guardian.

Authorization, Assumption of Risk & Release

Consent for Participation and Medical Treatment: I, the undersigned parent/guardian or student (if 18 or older), certify that the student is physically able to participate in the activities of the convention. In the event of injury or illness, I authorize the sponsoring organization and its agents to obtain and consent to emergency medical or surgical treatment, including hospitalization and anesthesia, as deemed necessary by licensed medical personnel. I agree to be financially responsible for all costs incurred as a result of such care.

Assumption of Risk and Release of Liability: I acknowledge that participation may involve inherent risks, including but not limited to travel, sports, demonstrations, and other activities. On behalf of myself and the student, I assume all risks and waive, release, and hold harmless the sponsoring organization, its officers, employees, volunteers, and agents from any and all liability, claims, demands, causes of action, or damages arising out of or related to the student's participation, except for willful misconduct or gross negligence.

Indemnification: I agree to indemnify and defend the sponsoring organization against any claims, damages, or expenses, including attorney's fees, that may arise from the student's actions or omissions while participating in convention activities.

Emergency Contact Authorization: If I cannot be reached, I authorize the named emergency contact or authorized medical personnel to make decisions regarding emergency care for the student.
