I, the undersigned, as the adult student or as the parent/legal guardian of the student named above, hereby give permission for the student to participate in the CPR/AED training described above. I understand the training includes practical, hands-on activities such as chest compressions, rescue breaths, and simulated use of training automated external defibrillator devices. I acknowledge that while reasonable precautions will be taken, participation involves physical activity and inherent risks of injury.

I certify that the student is physically able to participate in the training except as disclosed in the Medical / Emergency Information section. In the event of an injury or medical emergency occurring during the training, I authorize school personnel or program instructors to seek and provide emergency medical treatment as deemed necessary and to transport the student to a medical facility if required. I agree to be responsible for any costs associated with such treatment.

To the fullest extent permitted by law, I release and hold harmless the school, its board, officers, employees, instructors, volunteers, and agents from liability for injury, loss, or damage arising out of the student's participation in this training, except for injury resulting from gross negligence or willful misconduct by the released parties.
