Enrollment Agreement: By signing below I enroll the named student in the selected dance program. I agree to pay all tuition, fees and charges assessed. I acknowledge the deposit is non-refundable and that tuition remains due for the contracted term unless otherwise agreed in writing by program administration. I understand that failure to pay tuition may result in suspension from classes.

Refund and Withdrawal Policy: Withdrawals must be submitted in writing. Refund eligibility is limited and determined by the program’s published schedule; deposit and administrative fees are non-refundable. Pro-rated refunds may be available in limited circumstances at the program's discretion.

Attendance & Conduct: Regular attendance is required. Excessive absences may result in removal from performance opportunities or class reassignment. The program reserves the right to dismiss a student for disruptive or unsafe conduct without refund.

Assumption of Risk & Liability Release: Dance activities present inherent risks of injury. On behalf of myself and the enrolled student, I release, indemnify, and hold harmless the program, its employees, instructors, contractors, and agents from any claim, liability, loss, expense, or cause of action arising from injury, illness, or property damage, except to the extent caused by the program’s gross negligence or intentional misconduct.

Photo and Video Release: I grant the program the right to photograph and record the student during classes, rehearsals, performances, and program events and to use such images or recordings in promotional, educational, or archival materials. I understand no compensation is provided for use of likeness.

Emergency Medical Authorization: If a medical emergency occurs and I cannot be reached, I authorize the program staff to secure medical care and to transport the student to an appropriate medical facility. I agree to be financially responsible for any medical treatment provided.

Health & Safety: I agree to report any communicable illness or exposure in the household prior to attendance. I acknowledge that the program may implement health and safety protocols and that non-compliance may result in temporary exclusion.
