Attendance: Student is expected to attend all scheduled classes. Repeated unexcused absences may result in removal from the program. Makeup classes are permitted with prior notice and subject to instructor availability.

Late pick-up: Parent/Guardian understands a late pick-up fee of $ per minute will be charged for students not collected within 15 minutes of class end.

Photo / Video Release: I authorize the program to photograph and/or record the student and to use such images or recordings for promotional, instructional, or archival purposes without compensation. I understand images will be used at the discretion of the program.

Transportation: I authorize the student to be transported by authorized staff or contracted transport for approved off-site events. Specific trip permissions will be provided in writing for each event requiring transport.
