By signing below, Student authorizes the Delegate to act on behalf of the Student with respect to the specific matters checked and described. Authority is limited to the items checked and does not authorize actions beyond the text of this Agreement.

Limitations and Conditions

The Delegate is not authorized to make decisions that materially alter the Student's academic standing (such as withdrawal that requires Student's written consent), to consent to medical procedures beyond immediate first aid or emergency stabilization, to execute legally binding contracts on behalf of the Student except as expressly set forth in this Agreement, or to further delegate these powers. The institution may require the Delegate to present a valid photo identification before honoring any request.

The institution may, in its discretion, refuse to act on any instruction from the Delegate where the institution has reason to doubt the Delegate's identity or the scope of authority. The Student remains responsible for all obligations to the institution irrespective of actions taken by the Delegate under this Agreement.

Term, Revocation, and Notice

This delegation is effective from through unless earlier revoked in writing by the Student or the Student's legal representative.

Revocation must be delivered in writing to the institution's records office or other designated official. The institution shall act on a revocation upon receipt and reasonable opportunity to process it; the institution is not liable for actions taken by the Delegate before receipt of revocation.

Privacy and Acknowledgment

By signing this Agreement, the Student consents to the release of education records and other information to the Delegate as necessary to effect the delegated authorities. The Student understands that the institution will rely on this written authorization and that the Student may be required to provide additional proof of identity before records are released.

The Student and Delegate each acknowledge that they have read and understand the terms of this Agreement and accept the responsibilities described herein.
